Eligibility Requirements for Dogs


Operation PETS is an income qualified clinic. You must meet our eligibility requirements to use our services.

Pet owners must have a New York State Benefit Card from Department of Social Services for temporary assistance, Food Stamps or Medicaid to use our services
(see requirements below).

The ONLY exception is for Pit Bull/Pit Bull mixes (see requirements below).

Dogs adopted from an approved rescue organization/shelter are also eligible to use our clinic (see requirements below).


For your pet's protection we recommend dogs have their first DA2PPV (parvovirus-distemper) vaccine prior to coming to the clinic (vaccine take up to two weeks to take effect).

A $45.00 deposit is required before we can make an appointment and hold a surgical slot for your dog. You will only owe the balance on the day of your appointment. You can make your deposit over the phone via credit/debit card, by sending us a money order, or by coming into our clinic Monday-Thursday 10am-4pm. If you cancel or re-schedule your deposit is refundable as long as you give us at least 24 hours notice.

Approved rescue groups/shelters are exempt from deposits.

We do not require deposits from pet owners with dogs adopted from approved rescue groups/shelters if the spay/neuter surgery was included in the adoption fee (we bill the rescue group/shelter for payment).

NEW YORK STATE BENEFIT CARD FOR TEMPORARY ASSISTANCE, MEDICAID OR FOOD STAMPS

To be eligible, you MUST bring required documentation on morning of your appointment:

1. New York State Benefit ID Card
2. If your card does not have a picture must bring photo ID
3. If your card does not have issue date must bring proof that card is current (Approval Letter or Re-Certification Letter)
4. Proof of Ownership
5. Vaccination Records
6. If dog has current rabies vaccine we need to see the rabies certificate

PIT/PIT MIXES QUALIFY THREE DIFFERENT WAYS

1. NEW YORK STATE BENEFIT CARD FOR TEMPORARY ASSISTANCE, MEDICAID OR FOOD STAMPS

To be eligible, you MUST bring required documentation on morning of your appointment:

- New York State Benefit ID Card
- If your card does not have a picture must bring photo ID
- If your card does not have issue date must bring proof that card is current (Approval Letter or Re-Certification Letter)
- Proof of Ownership
- Vaccination Records
- If dog has current rabies vaccine we need to see the rabies certificate

2. RECEIVE HEAP (Home Energy Assistance Program)

Pit/Pit Mixes qualify if you receive HEAP. To be eligible you MUST bring required documentation on morning of your appointment:

- Proof of Ownership
- Photo ID
- HEAP Verification Letter or proof of HEAP
- Vaccination Records
- If dog has current rabies vaccine we need to see the rabies certificate

3. INCOME (HEAP ELIGIBLE)


Pit/Pit Mixes qualify if your total gross monthly income for your household size is at or below HEAP Income Guidelines
(www.heaphelps.com).

To be eligible you MUST bring required documentation on morning of your appointment:

- Proof of Ownership
- Photo ID
- Last two pay stubs (or)
- 2009 IRS 1040 & W-2 (Wage & Tax Statement)
- Vaccination Records
- If dog has current rabies vaccine we need to see the rabies certificate
DOGS ADOPTED FROM RESCUE/ADOPTION GROUP OR SHELTER

When calling for an appointment let us know the organization you adopted the dog from. If it is an approved rescue/adoption organization or shelter you are eligible to use Operation PETS.

Many organizations include the cost of the spay/neuter surgery in their adoption fee (we bill the rescue organization/shelter for payment). Some do not include the spay/neuter surgery and you are responsible for payment.

To be eligible you MUST bring the required documentation on morning of your appointment:

- Proof of Ownership
- Adoption Paperwork
- Vaccination Records
- If dog has current rabies vaccine we need to see the rabies certificate
