Operation PETS is an income qualified clinic. Cat owners qualify to use our clinic if they meet ONE of the eligiblity requirements below.
NEW YORK STATE BENEFIT CARD FOR TEMPORARY ASSISTANCE, MEDICAID OR FOOD STAMPS
To be eligible, you MUST bring required documentation on morning of your appointment. If you don't bring proof of eligibility you will not be eligible to use our services.
1. Valid New York State Benefit ID Card
2. If your card does not have a picture must bring photo ID
3. If your card does not have issue date must bring proof that card is current (Award Letter or Re-Certification Letter) RECEIVE HEAP (Home Energy Assistance Program)
To be eligible you MUST bring required documentation on morning of your appointment. If you don't bring proof of eligibility you will not be eligible to use our services.

2. HEAP Verification Letter or proof of HEAP INCOME (HEAP ELIGIBLE)
Qualify if your total gross monthly income for your household size is at or below HEAP Income Guidelines.

1. Photo ID
2. If employed must bring one month's worth of pay stubs
3. Unemployment Benefits - Must bring proof (award letter/certificate, monthly benefit statement from NYS Dept of Labor, printout of account information from NYS Dept of Labor’s website, copy of Direct Payment Card with printout)
4. SSDI (Social Security Disability Insurance) - Must bring Proof of Award Letter showing your benefit information
5. SSI (Supplemental Security Income) - Must bring Proof of Income Letter showing your benefit information. COLLEGE STUDENT
To be eligible you MUST bring required documentation on morning of your appointment. If you don't bring proof of eligibility you will not be eligible to use our services.
1. Current Student ID
2. If no picture on Student ID must bring photo ID
3. If no date on Student ID must bring proof of enrollment (class schedule or bill for current semester)
FERAL CAT
No income qualification required but must be an educated caregiver and follow our Feral Cat Protocol.MOM WITH KITTENS
Some moms with kittens qualify for our clinic (mom and kittens MUST all come to clinic for spay/neuter). Call us for more information on eligibility requirements.FRIENDS OF ANIMALS SPAY/NEUTER CERTIFICATE
Operation PETS accepts the Friends of Animals Spay/Neuter Certificate. However, we may restrict the number of surgery slots. The spay/neuter certificate is only valid for routine surgeries. The Friends of Animals certificate guidelines MUST be followed (www.friendsofanimals.org).
