Cats - Personal Pets
Clinic Eligibility Requirements


Operation PETS is an income qualified clinic. You must meet our eligibility requirements in order to use our services.

To bring a cat to our clinic pet owners MUST meet one of the following eligiblity requirements.

Income verification is required on the day of your appointment.

NEW YORK STATE BENEFIT CARD FOR TEMPORARY ASSISTANCE, MEDICAID OR FOOD STAMPS

Required documentation on day of your appointment:

1. New York State Benefit ID Card
2. Approval Letter or Letter of Re-Certification for Benefit Card
3. Photo ID

RECEIVE HEAP (Home Energy Assistance Program)

Required documentation on day of your appointment:

1. HEAP Verification Letter or proof of HEAP
2. Photo ID

HEAP ELIGIBLE

Qualify if the total gross monthly income for your household size is at or below the HEAP Income Guidelines (HEAP HELPS.COM)

Required documentation on day of your appointment:

1. Copy of last two pay stubs
2. Copy of IRS Form 1040
3. Copy of W-2 (Wage & Tax Statement)
4. Photo ID

COLLEGE STUDENT

Required documentation on day of your appointment:

Current Student ID and photo ID. If no date on Student ID must bring proof of enrollment, such as a class schedule or bill for current semester.

FERAL CATS

No income qualification required but must be an educated caregiver and follow our Feral Cat Protocol.

MOM WITH KITTENS

Most moms with kittens qualify for our clinic but mom and kittens must come in for spay/neuter on same day. Call us for more information on eligibility requirements.
