What To Bring To Your Appointment

On the day you made your appointment we reviewed our Eligibility Requirements with you. In order to use our clinic you must present your income/eligibility verification when you check-in. How did you qualify? Just click on the link below for details on what you need to bring.

501(C)(3) Rescue/Adoption Organization or Shelter

DOG - New York State Benefit Identification Card

DOG - Pit Bull/Pit Mix and Receive HEAP (Home Energy Assistance Program)

DOG - Pit Bull/Pit Mix and Meet HEAP Income Guidelines

CAT - New York State Benefit Identification Card

CAT - Receive HEAP (Home Energy Assistance Program)

CAT - Meet HEAP Income Guidelines

IF YOU ARE A 501(C)(3) Rescue/Adoption Organization or Shelter
You must have an approved Participation Application on file with us.

IF YOU ARE BRINGING A DOG and qualified with a New York State Benefit Card for temporary assistance, Medicaid or Food Stamps you must bring:
1. New York State Benefit Identification Card
2. Award or recertification letter from Department of Social Services
3. Photo ID
4. Proof of Ownership
5. If your pet has current vaccination records please bring them.
6. If rabies vaccine is current we need to see proof (rabies certificate)
7. Cash, credit or debit card

IF YOU ARE BRINGING A PIT BULL/PIT MIX and qualified because dog is a Pit/Pit Mix and you receive HEAP (Home Energy Assistance Program) you must bring:
1. HEAP Verification Letter or current utility bill showing HEAP
2. Photo ID
3. Proof of Ownership
4. If your pet has current vaccination records please bring them.
5. If rabies vaccine is current we need to see proof (rabies certificate)
6. Cash, credit or debit card

IF YOU ARE BRINGING A PIT BULL/PIT MIX and qualified because dog is a Pit/Pit Mix and you meet HEAP income guidelines you must bring:
1. Copy of last two pay stubs
2. Copy of 2008 IRS Form 1040
3. Copy of 2008 W-2 (Wage & Tax Statement)
4. Photo ID
5. Proof of Ownership
6. If your pet has current vaccination records please bring them.
7. If rabies vaccine is current we need to see proof (rabies certificate)
8. Cash, credit or debit card

IF YOU ARE BRINGING A FERAL CAT
1. There is no income qualification but you must follow our
Feral Cat Protocol.
2. Cash, credit or debit card

IF YOU ARE QUALIFIED WITH A MOM CAT AND KITTENS please call us for verification requirements.
