IF YOU ARE BRINGING A DOG and qualified with a New York State Benefit Card for temporary assistance, Medicaid or Food Stamps you must bring: 1. New York State Benefit Identification Card 2. Award or recertification letter from Department of Social Services 3. Photo ID 4. Proof of Ownership 5. If your pet has current vaccination records please bring them. 6. If rabies vaccine is current we need to see proof (rabies certificate) 7. Cash, credit or debit card IF YOU ARE BRINGING A PIT BULL/PIT MIX and qualified because dog is a Pit/Pit Mix and you receive HEAP (Home Energy Assistance Program) you must bring: 1. HEAP Verification Letter or current utility bill showing HEAP 2. Photo ID 3. Proof of Ownership 4. If your pet has current vaccination records please bring them. 5. If rabies vaccine is current we need to see proof (rabies certificate) 6. Cash, credit or debit card IF YOU ARE BRINGING A PIT BULL/PIT MIX and qualified because dog is a Pit/Pit Mix and you meet HEAP income guidelines you must bring: 1. Copy of last two pay stubs 2.
